I was just applying for an affiliates program (yes, they're lame, but
every little bit helps.) On the online application form, they asked
for the date my site was established and they insisted on month and
day, not just year!
How many people here actually remember the exact date their site first
went online? Geeze - I had to look at my copyright page to remind
myself what year it was started!

The date the site was established could be any number of things - is
that when the domain was first registered? When it was first added to
DNS? When the site was actually made live?
And, no - I wouldn't have a clue what date any of my sites started.

I would have put in something like "October 23, 4004 BC"

My sites are mostly domains and havent moved around much so the whois can
tell me. If that fails you can always check theWaybackMachine at
archive.org altho if you are like me you might NOT want to be reminded of
what your pages looked like years ago

I'm guessing they need that format to keep their database tidy!
Otherwise yes, a bloody stupid question.
